If you are actively experiencing homelessness (i.e. staying in your car, in a shelter, or sleeping outside) or are in a hotel, please call us for possible assistance:

Eviction Notice

If you are facing eviction and have received a notice to vacate in Polk County, please review the conditions below and then complete the pre-assessment.

To be eligible for possible services, the following conditions MUST be met:

  • The notice to vacate must be dated within in the last 30 days
  • The name on the notice must match the name of the person completing the application
  • All pages of your current lease agreement must be attached

Staying with friends or family

If you are currently staying with someone for one week or longer in Polk County, you are required to obtain a notarized letter. Please review the conditions of the letter below and then complete the pre-assessment.

To be eligible for possible services, the following conditions of your notarized letter MUST be met:

  • The current date
  • The applicant's full name
  • The friend or family member's address and phone number
  • A statement confirming that this is a temporary living arrangement and that you must vacate the residence
  • A date stating when you must vacate the temporary living arrangement

Next steps...

I've completed the pre-assessment, now what?

Following the completion of your pre-assessment with the correct documentation, a representative from the Homeless Coalition of Polk County will call you to complete an assessment over the phone, also known as, Coordinated Entry. Please keep in mind this assessment is not a housing assessment; it is a hardship assessment. It helps us to get an idea of your current situation and to determine what programs may work best for you, if you receive assistance.

I've completed coordinated entry, what's next?

After the completion of your hardship assessment the representative will explain to you that you are responsible for staying in contact with us within every 90 days. This is imperative to keep your file open in our database. In order to be possibly referred to a provider you must be open in the database. If there are any changes in your living situation, we also ask that you call or email us to inform us.

Other than staying in contact, we urge you to take a look at our resources and look for organizations that may be able to assist you as well. There are also organizations that may be able to fill in the gaps whether it be for food, clothing or healthcare.
